Do you have a long-term health condition?
If yes, you can self-refer to our Health & Well being Coaches who can provide you with one-to-one or group-based support to help you achieve your health-related goals. Their expertise will help you to:
- Take more of an active role in your health and wellbeing
- Access personalised support to identify and achieve your health-related goals
- Negotiate and overcome obstacles and barriers In your way

- Better control of eating habits without restrictions
- Better activity levels despite your challenges
- More motivation and confidence to get the most out of life

Please self-refer by emailing pics.mansffeldnorthwbc@nhs.net with your name, DOB, registered practice, and information relevant to your health and goals. A member of our team will then be in touch within 3-4 weeks of receiving your referral. By self-referring, you consent to sharing your details with Primary Integrated Community Services.
